I simple illusion using a flayed (skinned) finger prosthetic made of modelling wax and surrounded by fake skin. This is also known as a degloved finger. To add to the effect you could try Freakmo’s gelatine finger sheath found in her awesome degloving tutorial or surround the finger with random pieces of flesh coloured silicone and blood to simulate a hand trauma.
