Five of us, Allen Shah, Peter Donohue, Roger Schumann, Tony Johnson and Bill Vonnegut were reminded that communication can play a major factor when things go wrong. We had a capsize and swim on a recent trip to Cypress Point in Monterey CA, where it broke down and almost caused not only significant more time in the water for a swimmer, but very nearly more danger for the rescuer by making them think they had no choice but to make the run through some very big rocky surf to get the swimmer to safety.
