On May 24, 2021, LASD Industry Station were responding to a report of an apartment house fire in La Puente. During this response, an LASD CVPI was waiting on the corner of Willow Ave. and Amar Rd. for multiple LASD units responding to the fire to pass by code 3 as they had the right of way. Once the intersection was clear of LASD units, the CVPI entered the intersection code 3 with lights and sirens. Halfway into the intersection, the CVPI turned off the sirens, but the lights remained on. It was at this moment, a deputy involved traffic collision occurred at the intersection of Willow Avenue and Amar Road in the city of La Puente. Two cars collided with the CVPI at the intersection which caused the CVPI to do a 180 degree turn and sandwich itself between three vehicles. LASD and LACoFD responded to the traffic collision.
