Endoscopic anatomical enucleation of the prostate for BPH. The video demonstrates the “en-block“ technique using a bipolar instrument. This is a more advanced technique that allows to enucleate the whole adenoma without dividing the prostatic lobes. It is a straightforward enucleation where orientation and recognition of the correct surgical plane are even more important as there are not many anatomical landmarks (especially the internal urethral lumen is not a landmark in this technique). The direction of the enucleation is from the apex to the bladder neck. At the end the adenoma is morcellated and the tissue is send for pathologic evaluation. The “en-block“ technique is even more anatomical and elegant.
