I imagine, like many people, Swiftpaw being wracked with guilt after his death and the dismemberment of Brightheart. A monster with his friend's face harasses him in Starclan, reminding him of his mistake and the consequences. The monster is inspired by the creatures of Petit Poisson in these lost footage warrior cats (on youtube), which I highly recommend !
